Note: we cannot issue duplicate receipts or change the name after it is generated. If you with to donate anonymously, simply put "anonymous" in the name section.

Donor Information

A Tax Receipt for use on your tax return will be issued by Stage And Screen Innovations. The tax receipt will be issued, TO THE NAME AND ADDRESS PROVIDED, and WILL BE DELIVERED TO THE EMAIL ADDRESS PROVIDED. We can legally only issue ONE tax receipt per donation. If you wish to donate anonymously, please insert "anonymous" in the name area and leave the rest of the form blank. No tax receipt will be issued for anonymous donations.
